* Student/Trainee Registrations:
A letter of verification from a Department Head (on University/Hospital letterhead) is required to register at Student/Trainee rates. Please email your letter to the Conference Coordinator, Alex Slade (alex@hartleytaylor.co.uk)
* Retired Registrations:
Proof of age must be provided to validate ‘Retired (>70)’ registrations. Please email a copy of your Driving License, Passport or other valid ID to Alex (alex@hartleytaylor.co.uk)

Closing date for registration is Wednesday 17th February 2016. After this date you will only be able to register on-site at the conference.
Written notification of cancellations must be sent by post, fax or email to the Conference Coordinator. Cancellations will be accepted until 6 weeks prior to the meeting (Wednesday 20th January 2016) with a refund of your fee (less £50 for administrative costs).
The participant acknowledges that he/she has no right to lodge damage claims against the organisers should the holding of the congress be hindered or prevented by unexpected political or economic events or generally by force majeure, or should the non-appearance of speakers or other reasons necessitate programme changes. With registration, the participant accepts this proviso.
